At LeBauer, our primary care physicians emphasize the needs of the whole patient and partner with you to realize your best health. Our Primary Care group includes Internal Medicine Physicians who treat adults and Family Medicine Physicians who treat both children and adults.

Our Primary Care group also makes referrals to sub-specialists as needed, and are backed by the depth of the entire LeBauer network and the Cone Health System. It’s the difference between one physician looking out for your health and an entire network of physicians and sub-specialists at work for you and your family.

What We Treat

Many common diseases, such as hypertension, high cholesterol and diabetes, are preventable and treatable. Your LeBauer primary care physician specializes in preventative practices and can treat both acute and chronic health conditions.

General services include:
  • Immunizations (adults)
  • Same-day acute care
  • Minor office procedures: EKG, pulmonary function testing, minor skin procedures

FAQ

Are you taking new patients?

Yes, we are! Please request an appointment here.

What ages do you see?

Family Medicine sees patients of all ages, including infants. Internal Medicine varies from age 14 and up.

What should I bring to my appointment?

• Please bring all medicines you are taking, including the dosage. This includes vitamins, herbal therapies and all over-the-counter medicines. Please bring the actual bottles if possible.
• If you have records to transfer to us, please contact our medical records office at (336) 832-8677 or click here to learn about other ways to request your records from Cone Health.
• Current insurance cards must be presented.
• You will be responsible for your co-payment at the time of your visit. If you are not insured, please call the office to discuss your expected payment.

Please arrive 15 minutes prior to your appointment time so we can have time to process your insurance and paperwork.

What if I need to cancel an appointment?

Please allow a 24-hour notice when cancelling an appointment to avoid a $50 no show fee.

How do you handle prescription needs and refills?

Please let your nurse or doctor know at the time of your visit of any prescription needs. If you require refills, please contact your pharmacy at least five days before your medication runs out. This office requires at least three business days to process your medication orders.

NO CONTROLLED SUBSTANCES will be refilled after business hours. Be sure to call for your refills before you run out of your medication.

I need forms completed. Is there a fee associated with that?

If you have disability or FMLA forms, we will collect payment prior to completion of the forms. Please give us three business days for completion.

Do you take my insurance? What about Medicare or Medicaid?

We accept most insurances, but suggest that you contact your insurance provider to confirm that we are an in-network provider. Additionally, some insurances require your PCP to be updated prior to your visit to pay for services

We do accept Medicare. Medicaid is accepted on a limited basis, and excluding Carolina Access.

What are your regular hours of operation?

Our sites are typically open Monday through Friday from 8 a.m. – 5 p.m. Some of our providers offer early morning or evening appointments to accommodate your busy schedule. Please call your preferred location for availability.

What if I need care after regular business hours?

We want to make every effort to meet your healthcare needs, no matter the time of day. If you need care after regular business hours, please call your primary care office. By contacting your office, you will be directed to a nursing staff that can speak to you regarding your concerns.

Cone Health also offers virtual care options for convenient care any time of the day.

If at any time, you feel you are experiencing an emergency, please call 911 to reach emergency personnel.

Who do I contact for billing questions?

Why do I get two bills?

Although many insurance companies acknowledge the sick/well visit combination, some of them still requires you to pay two co-pays or have additional costs applied to your annual deductible.
