What are your hours of operation?

Our office is open from 7:30 a.m. - 4:30 p.m. Monday through Friday.

Do you accept my insurance?

LeBauer Neurology accepts most insurance plans. If you are uncertain if our office is a participating provider with your insurance plan, please call the customer service number on the back of your insurance card or contact our office for additional assistance.

What do I need to bring to my first appointment?

  • Your driver’s license or identification card
  • A list of your current medications or the medication bottles, including any over the counter medications
  • Your insurance card and co-pay (if applicable) (payment is expected at the time of your visit)
  • If you have had any testing (outside of the Cone Health system) related to your current diagnosis or if you have seen a neurologist in the past for this diagnosis, you will need to bring those reports and previous office notes to your appointment. This will allow our providers to have a more complete understanding of your medical history. You may bring those records and/or reports to your appointment or you may request they be faxed to us at (336) 832-3075.

Why do I have to show my insurance card at each visit?

Insurance must be verified at the time of service. Although your insurance may not have changed, it is important that we have verified that we have the correct and most up-to-date information entered in the system. This will save you, our patient, from receiving an incorrect statement for a denied claim.

How do I get a refill of my medication?

Patients may contact our office by telephone or contact your pharmacy to request a refill on your medications. Please allow two to three days for a refill request to be processed.

Are you accepting new patients?

Yes! Our office is currently accepting new patients, but we do require a physician referral. A member of our office staff will work with you and your referring provider to schedule an appointment to see one of our physicians.

What ages do you treat?

Our office treats patients 18 years and older. We do not accept pediatric patients.

What if I need care after regular business hours?

If at any time you feel you are experiencing an emergency, please call 911 to reach emergency personnel. We want to make every effort to meet your healthcare needs, no matter the time of day. If you need care after regular business hours, please call our office at (336) 832-3070. By contacting our office, you will be directed to our after-hours nursing service that can speak to you regarding your concerns.

What do I do if I am unable to make my scheduled appointment?

Please call our office as soon as possible if you are unable to make an appointment. Calling as soon as possible will allow us the opportunity to fill your appointment slot with another patient in need of neurological care. Any appointments not canceled or rescheduled at least 24 hours prior to your scheduled appointment time will be charged a no-show fee of $50 (for office visits) or $100 (for procedures/testing). Patients with multiple no shows are subject to discharge from our practice.
