After Hours Emergency:
A physician is always on call from each specialty after office hours and on weekends. The physician on call can be reached through our main number at 336-547-1700. Please follow the instructions. We cannot give prescription refills or lab results after hours.
If you are experiencing a life-threatening emergency call 911 for emergency medical services.
Appointments:
All office appointments are scheduled through our scheduling secretaries. You may call our main number (336) 547-1700 for appointments at our North Elam Avenue office or call the department you need directly. The main number has an automated attendant that will have instructions for contacting particular departments at the North Elam Avenue office.
Patients are seen by appointment only and every effort is made to stay on schedule. However, when emergencies arise, it may be necessary for you to wait beyond your scheduled appointment time. We will make every effort to keep you informed.
If you have a problem that requires immediate attention, but it is not an emergency, please call our office before coming and a physician will try to work you into his/her schedule as soon as possible. Please remember to call before coming in to the office.
As a courtesy to our patients our office utilizes a computerized appointment reminder system. This system will call you two to four days before your appointment to remind you of your appointment time however it is the responsibility of the patient to arrive on time for the appointment.
We try our best to schedule your appointment at the most convenient time possible. If you need to cancel or reschedule your appointment, we ask that you notify us by the end of business the day prior to your appointment, so that the time can be made available to another patient. Please be aware that failure to do so will result in a fee of $50.00 which is not covered by insurance.
What You Need to Bring For Your Visit:
Insurance information and all current insurance cards.
A list of medications (or actual bottles) with dosage and how frequently the medication is taken.
Any previous records if they pertain to your visit here.
Bring any other pertinent information such as test results or x-ray films from other offices, as requested.
If you are on a managed care insurance plan that requires an authorization for referrals to specialists, please bring your referral form from your primary care physician.
Fees and Payments:
We make every effort at LeBauer HealthCare to keep our fees fair and reasonable. Our policy is to collect any co-payments at the time of your visit. Please call Patient Accounting at 547-1877 to make payment arrangements, if necessary. Patients of LeBauer Endoscopy Center, a division of LeBauer HealthCare, (outpatient endoscopic procedures) will receive a facility bill in addition to the physician bill.
Insurance Information:
To assure accuracy, please make sure all your insurance information is up-to-date. Usually all the information needed is found on your insurance card. Please familiarize yourself with the coverage provided by your particular insurance plan. In addition, please bring your insurance card with you so that we can copy and file it in your chart. As a courtesy, our practice will file your insurance at no charge. However a signed authorization from you is requested in order to file charges. We participate in most managed care plans, i.e. HMO's and PPO's.
If you have a managed care insurance plan that requires referral authorization from your primary care physician, you are responsible for bringing the referral form from your primary care physician. If you do not obtain a referral authorization from your primary care physician, you may be responsible for the entire bill.
For our Medicare patients, our physicians accept "assignment" which means Medicare will pay us directly for 80% of the Medicare "allowable" charges. The patient is responsible to pay his/her annual deductible and 20% of allowable charges (or the co-payment). If a Medicare patient has supplemental insurance, we will file this insurance after receiving payment from Medicare. The patient is still responsible for the annual deductible. If Medicare is the secondary insurance, we will file it after we receive payment from the primary insurance.

Laboratory Services:
LeBauer HealthCare’s full-service, CLIA accredited laboratory is staffed by certified phlebotomists and technical professionals. Our extensive menu includes automated chemistry, immunoassay, and hematology testing, as well as manual assays. The laboratory, which also performs the high and moderate complexity tests on specimens obtained at our other offices, is housed in the main building on Elam Avenue. It is open to patients from 7:30 AM until 5:30 PM.
For patients’ convenience, phlebotomy service and limited manual testing is also available at the LeBauer HealthCare offices located in Asheboro, Stoney Creek, Guilford-Jamestown, Brassfield, and HeartCare on Church Street.
Prescription Refills:
If you need a prescription refill before a scheduled appointment, please call your pharmacy. Your prescription will be refilled much faster if you call your pharmacist directly. Your pharmacy will call LeBauer HealthCare for refill authorization. Please allow at least 48 hours for your refill to be processed. No refills for controlled substances will be authorized after-hours.
